easiest prepaid - most anonymous

Hello,
I want to get a visa/mc/amex that doesn't require some postal mail signup confirmation, or some sort of online check of my SSN and address. But I want something refillable - not a 'gift card'. The cards I have looked at so far in the stores have all sorts of privacy-invading policies. I live in a fairly large city so there's a number of retail outlets to choose from.

I am not a tax cheat or any criminal, I just want a couple of hundred bucks 'off the record', not attached to my SSN.
